In addition to the above analytical requirements the following shall be supplied for final acceptance:

• LSP opinion letter with a brief description of the project and overview of the sampling completed as well as conformance with Soil Management Plan. In addition, if conductivity testing was not completed briefly describe why not.

• Bill of Lading or Material Shipping record (signed by generator and signed and stamped by LSP)

• Site map indicating sampling locations and features of the site

• Table indicating analytical results compared to the requirements of Acceptance Criteria Table.

• Boring logs (or complete description) of the material proposed for Blue Hill Cemetery.

• Copy of all associated analytical and chain of custody identified on the table.
