In addition to the above analytical requirements the following shall be supplied for final acceptance:

• LSP opinion letter with a brief description of the project and overview of the sampling completed as well as conformance with DEP COMM 97-001. In addition, if conductivity testing was not completed briefly describe why not.

• Bill of Lading or Material Shipping record (signed by generator and signed and stamped by LSP)

• Site map indicating sampling locations and features of the site

• Table indicating analytical results compared to the requirements of Table 1 of COMM 97-001

• Copy of all associated analytical and chain of custody identified on the table.
